Please submit the following documents\/information for processing:<\/p>\n\n\n\n<p><strong> Guest\/student<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Name <\/li>\n\n\n\n<li>Email address&nbsp;<\/li>\n\n\n\n<li>Mailing address&nbsp; <\/li>\n\n\n\n<li>UMID (student only)<\/li>\n\n\n\n<li>Business purpose for purchases\/trip\/hosted meal<\/li>\n\n\n\n<li>Date of event<\/li>\n\n\n\n<li>Shortcode <\/li>\n\n\n\n<li>Itemized receipts showing <strong>proof of payment <\/strong>of the following:\n<ul class=\"wp-block-list\">\n<li>Lodging<\/li>\n\n\n\n<li>Airfare (Reimbursement is only for the cost of standard airfare. Flight should be <strong>booked at least  14 days<\/strong> in advance.)<\/li>\n\n\n\n<li>Hosted meals, including list of attendees<br><em>Note the following limits:<\/em>\n<ul class=\"wp-block-list\">\n<li>$30\/person for breakfast or lunch<\/li>\n\n\n\n<li>$70\/person for dinner<\/li>\n\n\n\n<li>$20\/person for any alcohol purchase; additionally, a separate shortcode must be used for the alcohol portion of the charge<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Transportation<\/li>\n\n\n\n<li>Mileage (Provide map of your trip with start point and end point.)<\/li>\n\n\n\n<li>Exception memo needed for expense restrictions.&nbsp;<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<p><strong>Employees<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Employees that need to be reimbursed for work-related expenses can submit this <a href=\"https:\/\/teamdynamix.umich.edu\/TDClient\/40\/Portal\/Requests\/TicketRequests\/NewForm?ID=yAjTx273jus_&amp;RequestorType=Service\">eForm<\/a> for reimbursement.
